Perhaps the difference is that "not guilty" means he walks free 100%, and "not guilty by reason of insanity" means no prison time, but he has to get psychiatric help.

If there no doubt as to whether or not this condition is the cause of the killing, I don't see how he shouldn't get court ordered mental help of some kind.
