Not necessarily. It depends on body type. Also "lolicon" refers to females. "Shotacon" or "Shota" is the male equivalent . . . .

That depends. "Underage" by what definition. If you mean two 15 year olds getting it on, and both of them are rather developed (girl has full figure, large breasts, etc), then that would be considered underage sexual relations by American standards, but not necessarily underage in other countries.

Furthermore, most people think of Lolicon as referring to characters who are both young and look young, usually to the point of being pre-pubescent...though the only loli's I ever like are the older, pubescent ones.
