The biggest ever would be "Bondage Game", where injection enhancement increases their size to maybe 4' across, so much so that the female can stand up, lean forward, and rest on them while being raped from behind. Although this goes against the law of conservation of matter & the second law of thermodynamics, it is entertaining in a bizarre sort of way. I think "Project Boobs" has the mutant "biggest boobs ever" (nowhere near Bondage Game), but it has been a while since I saw that.
