Where and when did Lilith show up, and how did they get her into Nerv without a working EVA? They said no angel had appeared for 15 years or so and that. Why is Rei injured in the first episode, anyway? Why is EVA-00 considered a prototype and Rei an active pilot if EVAs had never been deployed? How did NERV create all its dirty little projects if Gendo only became head of the organization in the first episode? Finally, are we expected to believe that all LCL in Terminal Dogma is came out of Lillith's... midsection, there? And why does she always look pregnant? I clearly missed something.

Lilith was in the GeoFront (the Black Moon) for the past 4 billion years or so.
Rei was injured in the EVA-00 activation test we see in flashback in episode 5
Gendo was running the NERV operations since 2010 - and Gehirn before that, from about 2003.
LCL seems to be Lilith continually bleeding primordial soup - a never ending heavy period.

Something shaped like a huge sphere crashed into Earth. She was inside. That something was called the Black Moon, and they essentially built NERV into the interior of it. It's mostly underground at the start of the anime.
They said no angel had appeared for 15 years or so and that. Why is Rei injured in the first episode, anyway? Why is EVA-00 considered a prototype and Rei an active pilot if EVAs had never been deployed?
EVA-00 is consdered the prototype because she is the first functional Evangelion that NERV managed to create. Rei is considered the an active pilot because she is capable of synchronsation with the Evangelion, and because she's one of a tiny number of people who could pilot EVA-00. As you probably know, a pilot is linked to his or Evangelion by, for want of a better term, genetic ties.
With regards to Rei's injuries, if you recall the flashback episode in which we were shown EVA-00 going berzerk, she was injured during that incident. The Entry Plug was thrown against the wall at high velocity, and the cushioning effect of LCL was not sufficient to compensate.
How did NERV create all its dirty little projects if Gendo only became head of the organization in the first episode?Gendo had already been the Commander of NERV for quite some time prior to the first episode. I'm quite certain of that.
Finally, are we expected to believe that all LCL in Terminal Dogma is came out of Lillith's... midsection, there?I'd say that we are indeed expected to believe this. I myself do believe that, and I regard that as being the only logical theory that accounts for all the observed data. She's been down there for quite some time, and then there are other factors, such as whatever NERV has done to her, and that accursed Spear. I don't even pretend to fathom the true nature of the Spear.
And why does she always look pregnant? I clearly missed something.I'll leave this one for Reichu. I think she may have some exceedingly interesting things to say on that topic, and I wouldn't wish steal any of her thunder.
Please do remember, though, that everything you need is right in front of you. If you have the complete series, with the director's cut of the latter episodes, and if you have the films, then there is nothing that anyone else can tell you, other than to point out details, links, explanations and correlations that you might have missed.

I'm simply trying to tell you that nobody on a forum can give you an answer that is automatically better than the one you arrive at yourself. You could be right, you could be wrong, but there is absolutely no ancient wisdom of which to avail yourself, just a bunch of other fans. ^_^
Did Adult Swim show the movies? If not, you should seriously consider a rental, or something. As for your question about the angels, Adam is first, and Sachiel is the third, which would seem to make Lilith the second. It wouldn't make a huge difference, that I can see, if they were numbered the opposite way around. It's also worth noting that the definition of "Angel" is a little bit flexible.

Specifically that line where they have Misato tell Shinji that Lilith was born from Adam...

This is true, of course. I just don't really like the notion that just because people post on this forum every day they are regarded as knowing that much more about Evangelion than the guy who just watched it. They all saw the same anime. They all put on their pants one leg at a time. We're all equal here.

The menstruation correlation is rather apparent, but, in a way, "birthing" is more accurate. All life 'born' from Lilith did so by way of LCL (with some notable exceptions), and that LCL came from her womb.
Also: Shinji reforms from LCL in EVA-01's core in #20, and Zero and Sho give us large LCL splatters from their cores in #23' and #26' respectively. Additionally, yellowish fluid that looks supiciously like LCL splurts out of Leliel when Shogouki shoots.

Those ideas are flimsy enough to fall under the "fanwank" stigma. What were the "exceedingly interesting things" you were thinking of?

If you look its dribbling out from behind her. Plus she has some pipes coming out of the back of her head into the cross labelled "LCL #1" etc.
I'd guess that the Flow of LCL coming out was what happened after the pipes sucked it out - it just dribbled into the lake.

My opinion is that if Shinji dissolved into LCL and was abosrbed into the core they are probably full of it - (but then shouldn't it leak out when the cores are stabbed?)

Shinji dissolved into LCL within the entry plug. During the salvage operation, the entry plug opened (don't really understand why) and all of the Shinji juice fell out. Shinji's soul found its way to Mommy and he reembodied from fresh Tang inside EVA-01's core, then :squirt:!

It leaks out when EVA-00's core cracks open. Does that count?

Perhaps only EVA cores have LCL in them then?
(The entry plug opens becuase they were pouring oxygen into it and stuff jammed and the pressure built up and *POP*and Shinji Juice everywhere and I use 'and' far too much)

It would have been funnier if Shinji had been expelled from Sho's core as bloody chunks of meat. The chunks would then spontaneously combine and form a half-rotten Evil Shinji, who would growl "I live again!" Then he would proceed to kick all sorts of Ash.
