Well, there's this Space Battleship Yamato game on the PS1... it's very strange. I loaded it up this one time, and basically spent about a half hour pressing the X button to clear the dialogue windows. It could have been longer. Time lost all meaning.
I just kept wondering "Has it started yet?", "Does it start soon?", and "Is this the game?!?!"
Not quite what you were looking for in an answer, I think, but truly it did confuse me...

When I found out that (.hack spoilers) Mia was part of the Wave and I had to fight her
When I found my first Oblivion gate in, you guessed it, Oblivion (the one at Kvatch).

I sorta did the same after paying money for it.
Oblivion has some pretty amusing bugs. I knocked a guard captain out, stood over him and pressed interact to hear him exclaim "Ah! It's the hero of Kvatch!" from the floor with his eyes closed.

A bunch of times at the near the end of Metal Gear Solid 2: Sons of Liberty.

The first Half-Life. You play what seems like forever, battle incredible odds, perform feats of dexterity, make it to the end, jump through the gate to Zen, and that's when you realize that you're only halfway through the game. I nearly cried.

I remembered another great one. In Final Fantasy III/6 when Kefka destroys the world and you're only half-way through the game.

The end of soul reaver 2 Raziel just kinda fades into the spectral realm, BOOOO!
Also, playing Unlimited SaGa, Devil May Cry 2, and Ephemeral Fantasia all made me go wtf.
Oh yeah, and all those horrid bugs in Burnout Revenge <_<. I won't even get started on those

Not to mention that at Xen is total grounds for your painful rape.
My WTF moment was in HL1 where I beat it half way and died to remember I didn't SAVE.I cried HARDER dess.:P

Apparently you've never heard of Silent Hill.
In regard to Half-Life and Xen, I'd say Xen constitutes maybe 20% of the game, it just seems like more because all of the stupid jumping/falling/teleportation/instant death things have you quicksaving and loading so much that you lose all sense of time and space.
And to dothacker, Half-Life autosaves every time you load a new area.

There's also the case of Ninja Gaiden (NES). Reasonably difficult, reasonably difficult, reasonab... Destroyed! The last level actually breaks into your grandmother's house and beats her at that point (and if she's dead, it digs her up. It's that much of a bastard).
I've always thought that deserved an enormous WTF! I've beaten the Xbox Ninja Gaiden on Hard, and yet the last level of the NES one makes me cry.

DDR/Stepmania when the song is going smoothly and then they drop all this matrix stuff on you. big WTF
Perfect Dark 1 first meeting Elvis
demo of Soul Reaver (whichever was on Dreamcast) he's a vampire yet you could only do things in the daytime and you die by water?! WTF
Silent Hill 1 made it near the end but didn't grab the map or something on the billboard. didn't know where to go and the town had already transformed, was low and health, and had already saved so everything was screwed. WTF I QUIT!
naturally anything dealing w/ Metal Gear Solid 2 and Killer 7

out of the 80 or so NES games i own, i've probably only beaten 3. could only make to 3rd level of Ninja Gaiden. i really sucked back then. i've beaten the xbox one on hard too, and you've reminded me that that game got its share of WTFs.

Legend of Zelda: Ocarina of Time after you pull out the sword and return to Hyrule castle. I was completely freaked out the first time I saw that everyone was a zombie, not to mention how the atmosphere in general changed from happy-go-lucky to creepy and hellish.

I think my first WTF moment was the same one as HitokiriShadow's in FFVI.
Another WTF was, when I bought Half-Life 2. I started it, installed it and
(no, now comes nothing about any problems with STEAM) I already knew
a little about the game because I downloaded the demo weeks before, which
was in english. I liked the english dubbing and looked forward to the full version, but what got was the totaly goofed up german version which I had
bought (as a German I buy games in Germany). I really expected to have the
english dubbing but got a totally bad german one. In this moment I really thought: WTF, how can they spoil me that game?
To your problems with understanding the MGS2 ending: I had none.

Probably when Aeris gets killed by Sephiroth. I know everyone and their grandma have played this game, but the spoiler is up for the unfortunate ones who haven't yet.
In Sonic Adventure 2, in the Chao world, when my almost-ultimate-chao decided to die. I guess I'm a bad parent :'(

I'm guessing its either they dont understand what really was happing on the Big Shell or its because it ends with a cliffhanger that as of now has yet to be resolved.
Thank God for MGS4!

I think that a lot of people just didn't like the whole S3 thing. I'm not so sure its that they didn't understand it but just that it was kind of out there and they didn't like it.

Probably the same thing they were thinking when they made FF9...and subsequently stole their primary plot device from Legend of Legaia. THAT made WTF. "Fog...again??" Also, that very-end boss after Kuja, Necron, made go "WTF? Where the F did he come from?"

My wtf moment was when I was playing Star Ocean second story and I realized I couldn't turn off that horrible voice acting, hearing "Looks like we won" after every battle was torture. Also from Metal Gear Solid 2 You can see Vamp in the background when Raiden is talking with Rose.

Zelda sacrifices herself to save Midna... less then half way through the friggen game. That was one hell of a "WTF" moment right there.

Gears of War:
-monster is running away during a chainsaw, and you just see it get split in half way across the screen. wtf?
-the first time i realized.. they have chainsaws too ><
oblivion:
-i've seen it take at least 5 minutes for a body to stop rolling down a hill or stop jiggling.
-a mage in single player was randomly being chased by 15+ gaurds and got completely raped when they caught up
-8 arrows to the head isn't death
FF7:
-1 limitbreak to kill sephiroth
Team Fortress:
-random flying heads/body parts (learned anatomy through this game)
GTA3:
-the unstoppable whirling flamethrower of doom. 6 stars of gg in 45 seconds.
-learning that you can fly with the tank by pointing the gun backwards and firing.
there are so many moments.. i'm not even going to list all the games i played when i used to live in japan. so many of them just made me go WTF?

VERY BIG disappointment there.
Being a major Rygar lover from the NES days, the PS2 one made me go "WTF" O.o. The PS2 one was good...but hella boring in its own way...and a Fetus/Hydra Dragon hybrid of a monster...WTH? O_o

Fighting Baal for the first time in Disgaea, and just, in general, playing Shadow of the Colossus, DoDonPachi, and Siren.
Honorable mention goes to 2 other Candidates:
*Me playing Melty Blood: ReAct Final Tuned last night with Akiha in arcade mode. Len is a pain in the *** to beat. And she wasn't even skilling me. SHE WAS CHEESING LIKE A DAMN HUMAN.
*When your enemy in Empire Earth decides to be a Zerg Russian and amasses a huge army out of nowhere to attack you with.

RE4. With the classic starting line "They lost stock and had to shut down."
Wow Capcom. What a way to end a multibillion dollar company that has headquarters larger than the US all over the world. And has been evil since game one, with one line. Fail.

Playing Killer7. Huge head trip. After all, three of the bosses that I've faced so far are a nutjob with an afro and tentacles, two businessmen with their heads blown open and who throw potato-like things at you, and a crazy Japanese anime schoolgirl who totes around a machine gun.
When I got near the end of FFX and saw what Yu Yevon was, I made a "wtf! not cool", if that counts. Really random battles with Star Wolf in Star Fox Command made me lol and wtf simultaneously. There's others, but those are the ones I remember.
